This Telehealth Informed Consent ("Consent") describes the nature, risks, and limitations of telehealth services provided through the Pathli platform. By using telehealth services through Pathli, you acknowledge that you have read, understood, and agree to the terms outlined below.
Telehealth is the delivery of healthcare services through electronic communications, including video consultations, phone calls, and asynchronous messaging. Through Pathli, you may be connected with independently licensed healthcare providers who can evaluate your health status, discuss treatment options, and, if clinically appropriate, prescribe medication.
Pathli Health, Inc. is a technology platform that facilitates access to telehealth services. Pathli is not a medical practice and does not provide medical care. All clinical services, including medical evaluations and prescribing, are performed by licensed physicians and clinicians who are affiliated with independently owned and operated professional practices.
If your provider determines that compounded GLP-1 medication is clinically appropriate for you, your prescription will be filled by a state-licensed 503A compounding pharmacy and shipped directly to you. Compounded medications are not FDA-approved. They are prepared based on individual prescriptions and include documented clinical differentiation as determined by your provider.
You agree to provide accurate and complete information about your medical history, current medications, allergies, and health status. Providing inaccurate or incomplete information may affect the safety and effectiveness of your care.
You are responsible for informing your primary care physician about any treatment you receive through our platform.
You must be physically located in a state where your provider is licensed at the time of your telehealth consultation.
Telehealth may offer several benefits, including: increased access to healthcare providers without the need for in-person visits; convenience and reduced travel time; culturally informed care from providers trained on South Asian metabolic health profiles; and timely follow-up and medication management.
You understand and acknowledge that telehealth has limitations compared to in-person medical visits, including:
Your provider cannot perform a physical examination. Diagnosis and treatment recommendations are based on the information you provide and may differ from an in-person evaluation.
Technical difficulties, including poor internet connectivity, may interrupt or prevent your consultation.
There is a risk that the information transmitted may not be sufficient to allow for appropriate medical decision-making.
Your provider may determine that telehealth is not appropriate for your condition and may recommend an in-person visit.
Medication is not guaranteed. All prescribing decisions are made by your licensed provider based on their independent clinical judgment. Not all patients will qualify for GLP-1 medication.
Your telehealth consultations are conducted through secure, HIPAA-compliant platforms. Your health information will be treated as protected health information and handled in accordance with applicable federal and state privacy laws.
Your medical information may be shared with: your prescribing provider; the compounding pharmacy fulfilling your prescription; and Pathli's technology and administrative teams as necessary to coordinate your care, subject to applicable privacy protections and Business Associate Agreements.

Telehealth is not appropriate for medical emergencies. If you experience a medical emergency, including but not limited to severe allergic reactions, chest pain, difficulty breathing, or thoughts of self-harm, call 911 or go to your nearest emergency room immediately.
Your participation in telehealth services is voluntary. You may withdraw your consent and discontinue telehealth services at any time without affecting your right to future care.
By using telehealth services through Pathli, you confirm that:
